Minimum Qualifications:
- Minimum of 1 year experience in nephrology
- Proven problem-solving skills and ability to multitask
- Excellent written and verbal communication skills
- Certified Medical Assistant from a nationally recognized organization
Job Description:
The Chronic Care Navigator at Phamily is responsible for supporting the development of patient-centered, team-based care within the nephrology domain. Working closely with the nephrologist, the Navigator utilizes the Phamily platform to manage patients' care efficiently and effectively.
The core focus includes organizing patient data, identifying unmet needs, engaging patients in their care, facilitating treatment interventions, and improving communication within the care team and patients.
Key Responsibilities:
- Understand nephrology requirements for coordinated population health
- Collaborate effectively with care teams
- Establish population-appropriate care processes
- Utilize the Phamily platform to support patients with chronic diseases
- Contribute to quality improvement efforts in population health
- Proactively manage target populations
- Ensure a positive patient experience through interactions and proper routing of needs
- Recognize and report data inconsistencies
- Participate actively in teamwork and meetings
- Comply with regulations and company policies
- Provide data for care team processes
- Monitor and correct patient attribution
- Perform duties as assigned
