Job Description
We are looking for a skilled Complex Care Social Worker (LMSW) to join our team at LifeBridge Health in Baltimore, MD. As a Complex Care Social Worker, you will play a crucial role in facilitating discharge planning for difficult-to-place, high-cost, and escalated patients. Your responsibilities will include using an escalation protocol to identify barriers to discharge, coordinating with the interdisciplinary team, and ensuring timely and cost-effective discharges in line with utilization review guidelines.
Requirements
- Seasoned professional knowledge with a Master's degree.
- Licensed Master Social Worker (LMSW) certification.
