INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position, based in Randallstown, MD at Northwest Hospital, requires day and evening shifts with weekend commitment. The role involves providing discharge planning coordination and intervention in collaboration with the clinical team and medical providers, aiming to promote patient wellness and efficient health service utilization among a patient population with complex health needs.

Responsibilities

• Coordinate and implement discharge planning with clinical and medical teams.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment for patients with complex health needs.

Requirements

• Bachelor's degree with specialized field knowledge.
• BSN required from CCNE accredited schools.
