INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join LifeBridge Health's Northwest Hospital team. In this full-time role with weekend commitment, you will work day and evening shifts to provide discharge planning coordination and intervention in collaboration with the clinical team and medical provider.

Responsibilities

• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment for a patient population with complex health needs.

Requirements

• Bachelor's degree in Nursing (BSN) from CCNE accredited schools.
• Basic professional knowledge in a specialized field.
• Prior experience in utilization review, care coordination, or case management is beneficial.
