Job Description
Curana Health, a provider-led organization focused on senior living communities, is seeking a Nurse Practitioner or Physician Assistant for a full-time position in Springfield, MA. In this role, you will provide direct patient care services within the Post-Acute Care setting, prioritizing high-quality care and autonomy for senior living residents. Responsibilities
- Conduct clinical interventional and episodic direct care visits.
- Evaluate and treat residents for changes in condition.
- Collaborate with physicians for overall medical management.
- Coordinate care with other disciplines and family members.
- Perform Annual Wellness Visits and other duties as assigned.
Qualifications
- Valid nurse practitioner or physician assistant license in the state of operations.
- Current board certification in the respective field.
- Masters Degree in Nursing or Physician Assistant Studies.
- Strong leadership, interpersonal, and problem-solving skills.
- Excellent written and oral communication skills.
