Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health. Responsibilities
- Provide clinical social work services to patients and families.
- Address common problems related to institutional living, financial concerns, family relationships, and more.
- Assist with discharge planning and counseling for end-of-life care.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
