Job Description

We are looking for a dedicated and compassionate Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a range of challenges from adjustment to institutional living to end-of-life support.

Job Type: Full-time

Location: Baltimore, MD

Shift: Day shift (8:00am-4:30pm)

Key Responsibilities:

• Conduct psychosocial assessments and provide counseling to individuals and families.
• Assist patients with financial concerns and discharge planning.
• Offer support and guidance on coping with medical diagnoses and end-of-life issues.

Requirements:

• Bachelor's in Social Work (BSW) required, Master's degree preferred.
• LCSW-C certification preferred.
• LGSW designation.
