Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common challenges. This full-time position is located at Levindale Hospital in Baltimore, MD.
Responsibilities
- Conduct psychosocial assessments and provide counseling.
- Assist patients with adjustment to institutional living and aging-related changes.
- Facilitate discharge planning and provide support related to death and dying.
- Collaborate with healthcare teams to address financial concerns and family relationships.
Qualifications
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
