Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health, one of Maryland's largest healthcare providers. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments to address common problems.
- Assist patients with various challenges related to their medical conditions.
- Participate in discharge planning and provide counseling related to death and dying.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- LCSW-C certification preferred.
- LGSW certification required.
