Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges they may face. This role involves assisting individuals with adjustment to institutional living, financial concerns, family relationships, medical procedures, aging-related changes, discharge planning, and end-of-life counseling.
Responsibilities
- Provide clinical social work services and psychosocial assessments to patients/residents and families.
- Assist individuals with various common problems they may encounter.
- Collaborate with a multidisciplinary team to address patient/resident needs.
- Ensure compliance with regulatory requirements and ethical standards.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Excellent communication and interpersonal skills.
