Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. The ideal candidate will provide clinical social work services and psychosocial assessments to patients, focusing on various challenges such as adjustment to institutional living, financial concerns, and family relationships.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and end-of-life counseling.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
