Job Description
We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing numerous challenges including adjustment to institutional living, financial concerns, family relationships, and discharge planning. Your role will be crucial in supporting individuals and families through difficult situations and helping them navigate complex healthcare processes, fostering a sense of empowerment and well-being.
Responsibilities
- Provide clinical social work services to patients and their families.
- Conduct psychosocial assessments and identify areas of concern and need.
- Offer counseling and support related to various emotional and practical challenges.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Strong communication and interpersonal skills.
- Compassionate and empathetic approach to patient care.
