SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various common challenges such as adjustment to institutional living, financial concerns, and counseling related to end-of-life issues.

Responsibilities

• Deliver clinical social work services to patients and families.
• Conduct psychosocial assessments and provide counseling on various issues.
• Assist with discharge planning and end-of-life counseling.
• Collaborate with healthcare teams to support patient well-being.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, LGPC certification.
• Excellent communication and interpersonal skills.
