Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as institutional living adjustments, financial concerns, and counseling related to medical procedures and end-of-life care.

Responsibilities

Provide clinical social work services and psychosocial assessments.
Address common patient and family concerns related to health, aging, and end-of-life issues.
Assist in discharge planning and provide counseling and support.

Requirements

Bachelor's degree or equivalent with 1-3 years of experience.
LBSW, LMSW, or LGPC certification.
Strong knowledge and practice in social work theory.
