Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments.
- Provide counseling and support to patients and families.
- Assist with discharge planning.
- Collaborate with interdisciplinary teams.
Requirements
- Bachelor's degree in Social Work.
- 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Strong communication and interpersonal skills.
