INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients. The role focuses on promoting patient wellness, improving care outcomes, and ensuring efficient health service utilization.

Responsibilities

• Coordinate discharge planning in collaboration with the clinical team and medical providers.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients with complex health needs.

Requirements

• Bachelor's degree in a relevant field.
• BSN required from CCNE accredited schools.
• Prior experience in a similar role is a plus.
• Strong knowledge of discharge planning processes.
