Genesis HealthCare, a leading healthcare provider, is seeking an LPN MDS Coordinator to join their team. In this role, you will be responsible for participating in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and reimbursement.

Position Highlights

• Direct the RAI process and ensure accurate completion and transmission of MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train interdisciplinary team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license.
• Prior experience in healthcare settings.
• Familiarity with nursing practices and regulations.
• Computer proficiency, including AccuMed.
• Experience in MDS completion and Care Plans.
• BLS/CPR certification.
