Job Description
We are looking for a dedicated Social Worker to join our Care Management team at Carroll Hospital, part of LifeBridge Health. This part-time position requires a weekend commitment and offers a day shift from 9:00 am to 5:30 pm.
Summary
The primary role of the Social Worker is to collaborate with the healthcare team to ensure efficient inpatient care, prevent delays, and coordinate with community agencies on various social issues such as abuse, neglect, substance abuse, mental health, and adoption.
Requirements
- Master's degree in social work is required.
- Minimum of 2-4 years of recent social work experience, preferably in a medical setting.
- LCSW-C license is preferred, although LMSW licensed candidates will also be considered.
