SOCIAL WORKER PRN

Job Description

We are looking for a passionate Social Worker PRN to join our team at LifeBridge Health. As a key member of our allied health department, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and support during difficult situations.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of experience in a similar role.
• Licensed Social Worker (LBSW/LMSW/LGPC) preferred.
