Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health. As a SOCIAL WORKER, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling related to medical conditions and aging issues.
• Assist with discharge planning.
• Offer support for families facing difficult situations.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
