SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker (PRN) to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common issues related to institutional living, family relationships, aging, and end-of-life care.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and address financial concerns.
• Offer support to individuals facing medical challenges and aging-related issues.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as a Licensed Social Worker (LBSW/LMSW/LGPC).
• Strong knowledge of social work theory and practice.
• Ability to work with diverse populations and maintain confidentiality.
