Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health. This position involves providing clinical social work services and psychosocial assessments to patients and their families, addressing various common issues such as adjustment to institutional living, financial concerns, and family relationships. Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address a wide range of social and emotional issues faced by patients.
• Assist with discharge planning and counseling related to death and dying.

Requirements

• Bachelor's degree and 1-3 years of experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Knowledge of theory and practice within the field of social work.
