Job Description
We are looking for a passionate and dedicated Social Worker (PRN) to join our team at LifeBridge Health, specifically at Levindale Hospital in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common problems related to their healthcare journey.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with adjustment to institutional living and address financial concerns.
- Develop discharge plans and provide counseling related to end-of-life issues.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's degree in Social Work.
- 1-3 years of relevant experience.
- Licensed as LBSW/LMSW/LGPC.
- Strong understanding of psychosocial theory and practice.
