SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Assist patients and families in coping with medical procedures, aging-related changes, and other challenges.
• Collaborate with healthcare teams to develop and implement care plans.
• Offer counseling and support to individuals facing end-of-life issues.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal and communication skills.
