In this role, you will provide crucial clinical social work services and psychosocial assessments to patients/residents and families facing various challenges. Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address common problems such as adjustment to institutional living, financial concerns, family relationships, and more.
• Assist with discharge planning and provide counseling related to death and dying.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
