Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join LifeBridge Health in Baltimore, MD. As part of the SOCIAL WORK-HOSPITAL team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges related to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling and support for patients and families facing medical and end-of-life issues.
• Assist with discharge planning and resources for patients transitioning from the hospital.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
