SOCIAL WORKER PRN

Job Description

We are looking for an experienced SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. As part of our Allied Health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients/residents and families dealing with various challenges.

Responsibilities

• Offering counseling and support services to patients and families on adjustment, financial concerns, family relationships, aging, discharge planning, and end-of-life issues.
• Conducting psychosocial assessments to identify needs and develop care plans.
• Collaborating with healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
