SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and experienced Social Worker (PRN) to join our team at LifeBridge Health. As a PRN Social Worker at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common issues including adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and assessments to patients/residents and families.
• Address issues such as adjustment to institutional living, financial concerns, and counseling related to death and dying.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Conduct discharge planning and assist with transitions.

Requirements

• Bachelor's degree and 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
• Excellent communication and counseling skills.
