SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common concerns such as adjustment to institutional living, financial issues, family relationships, and counseling on end-of-life matters.

Responsibilities

• Conduct psychosocial assessments and provide counseling to address patient and family needs.
• Create discharge plans and assist with end-of-life care planning.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and updated patient records.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
