SOCIAL WORKER PRN

Job Description

We are looking for a passionate and dedicated PRN Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges related to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Conduct psychosocial assessments and provide counseling to address patient/resident and family needs.
• Assist with discharge planning and offer support related to end-of-life care.
• Collaborate with healthcare teams to ensure comprehensive care for patients and families.
• Maintain accurate and detailed documentation of patient interactions and progress.

Requirements

• Minimum of a Bachelor's degree in Social Work.
• 1-3 years of experience in a similar role.
• Licensed as a LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
