SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients, families, and residents, addressing a range of common issues such as adjustment to institutional living, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Offer clinical social work services to patients, residents, and families.
• Conduct psychosocial assessments and address various emotional and social challenges.
• Assist individuals in understanding medical diagnoses, procedures, and treatments.
• Provide counseling and support in areas like discharge planning and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
