SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker (PRN) to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services to patients and their families, addressing a wide range of psychosocial needs. From adjustment to institutional living to counseling on death and dying, you will play a vital role in supporting individuals through challenging times.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Offer counseling and support for various issues including financial concerns and family relationships.
• Assist with discharge planning and end-of-life care.

Requirements

• Bachelor's degree with 1-3 years of experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
