SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families. This includes addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common issues such as adjustment to institutional living and family relationships.
• Offer counseling related to medical diagnoses, aging, and end-of-life planning.
• Engage in discharge planning and provide support to patients and families.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong knowledge of social work theory and practice.
