Job Description

We are seeking a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. Your responsibilities will include addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Key Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common challenges faced by patients/residents and families.
• Engage in discharge planning and bereavement counseling.
• Collaborate with interdisciplinary teams to ensure comprehensive care.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong communication and interpersonal skills.
