SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, focusing on various common issues such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and their families.
• Address issues related to adjustment, financial concerns, family dynamics, and end-of-life care.
• Assist with discharge planning and support patients through difficult transitions.

Requirements

• Bachelor's degree in Social Work and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in clinical social work.
• Knowledge of social work theory and practice in a healthcare setting.
• Compassion, empathy, and strong communication skills.
