Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a part of our Allied Health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families. This role involves addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Assess and assist with common psychosocial issues related to medical treatments and aging.
• Engage in discharge planning and counseling for end-of-life care.

Requirements

• Bachelor's degree and relevant professional knowledge.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
