SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling. Responsibilities

• Offer clinical social work services to patients, residents, and families.
• Conduct psychosocial assessments to address a range of issues.
• Assist individuals in coping with medical conditions, aging, and discharge planning.
• Provide counseling on end-of-life matters and bereavement support.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
