SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN (as needed) Social Worker to join our team at LifeBridge Health in Baltimore, MD. This role involves providing clinical social work services and conducting psychosocial assessments for patients/residents and their families. The primary focus areas include adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling on end-of-life issues.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address various challenges such as financial concerns, family relationships, and discharge planning.
• Provide counseling support for end-of-life matters and powerlessness.

Requirements

• Basic professional knowledge with a Bachelor's degree and specialized field practice.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
