SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges they may face. This role involves working with patients on issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist patients in coping with medical diagnoses, procedures, and treatments.
• Develop discharge plans and provide support related to end-of-life care.
• Collaborate with healthcare professionals to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience as a Social Worker.
• Licensed as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
