Job Description

We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, focusing on various common issues such as adjustment to institutional living, financial concerns, and family relationships. You will also offer support in areas like discharge planning, end-of-life counseling, and aging-related changes.

Responsibilities

• Conduct clinical social work services and psychosocial assessments.
• Address common patient and family concerns related to medical diagnoses, aging, and end-of-life issues.
• Collaborate with healthcare teams to provide comprehensive care.
• Ensure patients receive appropriate support and resources for their needs.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC licensure.
• Strong interpersonal and communication skills.
