Job Description

We are looking for a compassionate and skilled Social Worker PRN to join our team at LifeBridge Health. As a PRN Social Worker in Baltimore, MD, at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, and end-of-life issues.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address common issues including adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
• Collaborate with healthcare teams for discharge planning and support.

Requirements

• Basic professional knowledge equivalent to a Bachelor's degree.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
