Job Description

We are seeking a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they face during their healthcare journeys.

Responsibilities

• Offer psychosocial support to patients and families facing adjustment difficulties, financial concerns, and end-of-life issues.
• Provide counseling on medical procedures, aging-related changes, and discharge planning.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of social work experience.
• LBSW/LMSW/LGPC certification.
• Strong interpersonal and communication skills.
