SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face. The position involves assisting individuals with adjustment difficulties, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Address adjustment issues related to institutional living and aging.
• Assist with financial concerns and discharge planning.
• Offer counseling on medical procedures, treatment acceptance, and end-of-life issues.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of experience in a relevant field.
• LBSW/LMSW/LGPC certification.
