SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services, conduct psychosocial assessments, and offer counseling to patients, residents, and their families on a range of common issues. As a vital member of our Allied Health team, you will address various concerns such as adjustment to institutional living, financial matters, family dynamics, medical understanding, aging-related challenges, and end-of-life support.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Offer counseling and support on adjustment to institutional living, financial concerns, family relationships, medical issues, aging, powerlessness, discharge planning, and end-of-life matters.

Requirements

• Bachelor's degree with 1-3 years of related experience.
• Licensed as LBSW, LMSW, or LGPC.
• Demonstrated knowledge of social work theory and practice.
