Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing a range of common issues. This position involves counseling, discharge planning, and support for patients and families during challenging times.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients/residents and families.
• Address various challenges such as adjustment to institutional living, financial concerns, and end-of-life issues.
• Collaborate with medical professionals to ensure holistic patient care.
• Participate in discharge planning and provide support during difficult transitions.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
