SOCIAL WORKER PRN

Job Description

We are looking for a compassionate SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. As a valued member of our Allied Health team at Levindale Hospital, you will provide important clinical social work services and psychosocial assessments to patients, assisting with a variety of common challenges they may face. This includes but is not limited to adjustment to institutional living, financial concerns, family relationships, medical understanding, aging issues, discharge planning, and grief counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address common problems related to adjustment, finance, family, medical understanding, aging, powerlessness, and death and dying.
• Collaborate with healthcare teams to ensure the well-being of patients.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• A strong understanding of social work theory and practice.
