SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to care, financial concerns, family relationships, aging-related changes, discharge planning, and counseling for end-of-life issues.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and support patients in dealing with medical diagnoses.
• Collaborate with healthcare teams to address patients' social and emotional needs.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a clinical setting.
• Licensed as LBSW, LMSW, or LGPC.
