Job Description
We are currently seeking a passionate Social Worker to join our team at LifeBridge Health. This is a PRN position located in Baltimore, MD at Levindale Hospital. The role involves providing clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments.
- Provide counseling and support to patients and families.
- Assist with discharge planning.
- Address issues related to aging and end-of-life care.
Requirements
- Bachelor's degree in Social Work.
- 1-3 years of relevant experience.
- LBSW/LMSW/LGPC certification.
- Strong knowledge of social work theory and practice.
