Job Description
We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position at Northwest Hospital in Randallstown, MD, involves weekend commitments with day and evening shifts. The Inpatient Case Manager collaborates with the clinical team and medical provider to facilitate discharge planning coordination and intervention, focusing on promoting patient wellness, improving care outcomes, and ensuring efficient health service utilization.

Requirements
- Bachelor's degree in Nursing (BSN) from CCNE accredited schools or equivalent.
